A meeting notes template for Excel: one row for each meeting.
The notes-only setup has the Meetings sheet alone: date, title, who came, purpose, notes, what was decided and the date of the next meeting. There is no Action Register, Decisions Log or Summary, so the file stays short and suits someone who only needs a record.

Questions
Are my builder entries sent to Due Form?
No. The builder uses them in your browser to make the workbook on your device.
Information is sent to Due Form when you submit a form or use another service described on the How it works page.
Can I add actions later?
Not to the same file. Make a new workbook with the Action Register switched on, then copy your meeting rows into it.
How do I find an old meeting?
Use the filter arrows in the header row, for example on Meeting or Date. Long notes wrap within the cell.
