Annual performance review prep for Excel, written through the year.
The annual setup covers a twelve-month period with five areas: Delivery, Quality, Teamwork, Customers and Learning. Objectives have a self-rating, feedback is dated and attributed, and a Development sheet lists each skill with how you will build it and by when.

- Name the areasEnter the headings your review uses, one per line, such as Delivery or Teamwork. They become the Area dropdown on the Objectives and Feedback sheets.
- Pick period and sheetsPick three months, six months or a year. Switch the Feedback sheet, the Development sheet and a self-rating on or off, and add up to eight columns of your own.
- Download and fill inDownload the workbook, list your objectives with how each is measured, and add feedback as you receive it. The Summary shows the days left to the review.

Questions
Are my builder entries sent to Due Form?
No. The builder uses them in your browser to make the workbook on your device.
Information is sent to Due Form when you submit a form or use another service described on the How it works page.
Can I rename the five areas?
Yes. Enter your own headings before you download, or edit them later on the Lists sheet, so the dropdowns match the competencies your employer uses.
Is the self-rating required?
No. Leave it blank, or switch the column off in the builder.
