Prepare objectives and feedback for a review.

Build a workbook for preparing a performance review. An Objectives sheet lists each objective with how success is measured, a due date and a status. A Feedback sheet records what people said, from whom and when.

  • Excel 2016 and later
  • Calculations checked in LibreOffice and Google Sheets
  1. Name the areasEnter the headings your review uses, one per line, such as Delivery or Teamwork. They become the Area dropdown on the Objectives and Feedback sheets.
  2. Pick period and sheetsPick three months, six months or a year. Switch the Feedback sheet, the Development sheet and a self-rating on or off, and add up to eight columns of your own.
  3. Download and fill inDownload the workbook, list your objectives with how each is measured, and add feedback as you receive it. The Summary shows the days left to the review.

JavaScript is required to use the builder. The workbook is made in your browser.

What you get

  1. Written as it happens

A row for each check-in: the date, what happened and the progress against the objective.

  2. Linked to each objective

Check-ins count against their objective, and any objective with none yet is listed before the review.

  3. Days to the review

Set the period and the review date on Settings, and the Summary counts down and prints on one page.

  4. Your own record

For an annual review, probation, a promotion case or regular one-to-ones.

The Annual review example

Apple Numbers
Some calculations do not workMany calculations on the Summary sheet and the Objectives sheet do not work in Apple Numbers. Numbers converts list-based dropdowns to plain cells and removes colour markers, sheet protection, frozen headings and number and date validation. Use Excel for these calculations and features.

Questions

Are my builder entries sent to Due Form?

No. The builder uses them in your browser to make the workbook on your device.

Information is sent to Due Form when you submit a form or use another service described on the How it works page.

How the utilities work

Can I print the Objectives sheet for the meeting?

Yes. Each sheet is set to print landscape on A4 or US Letter, one page wide, so the objectives and how each is measured can go on the table in front of you.

Who sees what I write?

Access depends on where you save the file and your spreadsheet app’s sharing settings. Share a copy or printed sheets with your manager when needed.

What are the review period dates?

The period starts on the first of this month and ends after the months you chose. Change both dates on the Settings sheet, and the Summary counts feedback inside that period.

Can I link feedback to an objective?

Yes. On the Feedback sheet, pick the objective from a dropdown that lists the IDs and wording from the Objectives sheet, so each comment is linked to the objective it is about.
