The check-in setup has a three-month period and four areas: Priorities, Wins, Blockers and Growth. Objectives are the things you agreed at each meeting, and the Development sheet records what you want to learn, how and by when. There is no Feedback sheet or self-rating.

Employees and managers who meet every few weeks and want a short running record of what was agreed instead of a yearly write-up.
- A three-month review period, so the Summary counts down to the next review.
- Areas suit a short meeting: Priorities, Wins, Blockers and Growth.
- The Development sheet is included; Feedback and self-rating are left out.

Questions
Are my builder entries sent to Due Form?
No. The builder uses them in your browser to make the workbook on your device.
Information is sent to Due Form when you submit a form or use another service described on the How it works page.
Can I use it for a team member I manage?
Yes. Make one workbook for each person. The objectives are what you agreed together, and the Status column shows which are On track, At risk or Done.
Can I add a Feedback sheet later?
Not to this file. Make the file again with Feedback switched on, using the same areas, then copy your Objectives rows into the new workbook.
